Cannot add MSSQL Database

When I add the database I get a error message "Error : 73100080 : Unknown exception occurred while adding SQL Server database."
 
When I added the DB server in order to test the connectivity to the database I had to include the port number with the instance name of the database. EG. my-server,1111 which worked, but once I add the role to the server the comma separated port is removed from the  Server name.
 
Also after a period of time the DB server is no longer available and shows it is down even though the server is up and running.
 
Is there a setting that I am missing?
Why is the DB also showing DOWN when it is actually still running?
