Login as HostAdmin and check what are the virtual directory settings at My Server :: Admin Virtual Directory Settings. If you've made any changes in account setup template then go to location My Server :: Customize Templates :: My Templates :: Edit Template and select the "Account Setup" and click at Edit Template and then click at 'Restore Default Template'.
Follow these two steps, see if this resolve the issue.
